School medical record
Every pupil/student has a school medical record, in which medical observations are documented by healthcare professionals, at their discretion. The healthcare professionals that see the pupil/student will document any information that is required for the purpose of monitoring the pupil's/student's health. With regard to psychological and academic information, only the date of the meeting with the relevant professionals is mentioned.
The school medical record is created when the first health check-up is done. It accompanies the pupil/student throughout their school career, and ensures continuity between the various school medical tests and examinations.
When the pupil/student leaves the education system, their school medical record will be handed over to them – or to the person with parental authority for them, if they are minors – on request.
School medical file
The school medical record forms part of the school medical file, which also includes confidential questionnaires, copies of vaccination records, medical opinions, BMI (body mass index) curves and, sometimes, reports from specialists, or the results of laboratory analyses.
The school medical file is confidential and is entrusted to paramedical personnel on the school medical and social support team. If the child moves home or changes school, their file is forwarded to the relevant school medical and social support team, which will continue socio-medical monitoring.
